She was preceded in death by James Buchanan and John Johansen and parents Leonard and Elaine Forti and sister Louise Ann Forti.

Sara is survived by daughters – Elaine Kinsey (Jerry) and Susanne Moghadam, Granddaughters Tara Lee Plank (Phillip), Sara Moghadam, Grandson – Adam James Moghadam (Kathy) – Great Grandsons – Elijah Lee Plank and Noah James Moghadam, Sisters – Camille Tower (Vincent) and Elaine Dentico (James) of Buffalo, New York , Brothers – Leonard (Carol) Forti of Atlanta and Robert Forti (Angie) of Nashville, TN,

There is currently no visitation planned. Friends may call or visit at the home of Elaine Kinsey, there will be a small family gathering at the home of Sara Johansen, Tuesday afternoon 4 to 7, friends of the family is welcome. There will be a funeral service in Buffalo, New York at a future time.
